A national hospital network wanted an AI solution to draft clinical notes from patient visits and give clinicians back the hours they lose to paperwork. Sending patient conversations to a cloud service was never an option, because protected health information cannot leave the network's control. With the Teknikos Hybrid AI Method, the network identified and shipped a hybrid solution that drafts each note on the clinician's own device, where the patient's data never leaves, and returns clinicians to the work only they can do.

  • The Situation

    Clinicians at the network spend hours every day on documentation, time taken directly from patients. The network wanted an AI solution to draft the clinical note from the visit itself, so a clinician could stay present with the patient and leave the writing to the machine.

    One fact ruled the project from the start: the visit is protected health information, and it cannot be sent to an outside service for a frontier model to read. A cloud solution would have put the most sensitive data the network holds in exactly the place it is bound to keep it out of. So the capability that would have helped clinicians most was the one they could not touch.

  • How the Method Worked

    The Teknikos Hybrid AI Method started by qualifying the use case. Its Eight Forces established that the work belonged on the device because the patient's data could not, by law, leave the network's control.

    With the Unified Platform, the solution was mapped to the Microsoft products and services that would run and govern it, on the devices clinicians already used and the management plane IT already operated.

    Its Hybrid Routing Tree determined the cloud and local split: the visit is captured and the note is drafted on the device, where the patient's data stays, and only a de-identified draft may cross to a frontier model in the cloud to be structured and coded before it returns to the record. The raw encounter never leaves the clinician's device.

    Its Operating Wheel set the plan to keep the drafting accurate and compliant as clinical models improve and regulations change.

  • The End State

    Clinicians got a drafted note from every visit, and the hours once lost to paperwork went back to patients. The most sensitive data the network holds never left the clinician's device, so a capability that had been off the table became one the network could run everywhere it delivers care. The whole solution runs on the Microsoft estate the network already operated, with compliance, security, and clinical leadership aligned behind it.
